Discrimination Harassment of a Student Including Sexual Harassment
Form to use: Uniform Complaint Procedure form
Description of complaint: This type of complaint is for use by any student who feels that he/she is being harassed, discriminated against, or intimidated with respect to a student's actual or perceived sex, gender, ethnic group identification, race, national origin, religion, color, physical or mental disability, age or sexual orientation. Any student who observes an incident of discrimination, harassment, intimidation, or bullying should report the harassment incident to a school employee, whether or not the victim files a complaint. Complaints of discrimination, harassment, or intimidation of a student shall be investigated and resolved in accordance with site-level grievance procedures and/or Uniform Complaint Procedures.
Where to file: With a school official of the school site where the incident occurred. A complaint may also be filed in accordance with Uniform Complaint Procedures listed above.
